Inadequate Training Can Be Fatal

The importance of workplace training was never more evident than in the recent report released by NASA on the Columbia shuttle disaster.
According to the report, the astronauts didn't have some of the training and equipment necessary to survive the disaster. While the crew was wearing pressurized suits, one did not have a helmet on and three were not wearing gloves. Their parachutes must be activated manually, so they are useless if the crew is unconscious. The crew knew for 41 seconds that they had lost control of the spacecraft.
The report notes that the crew was trained to solve flight problems, NOT to SURVIVE them. The report includes recommendations for the design of future spacecrafts and that astronauts of future missions should have automatically deploying parachutes.
Almost six years after the fatal flight, these are hard admissions to swallow and just reinforce the importance of workplace training--where ever that
